If you are a new patient you will be asked to complete a medical and dental health form. For your convienance you can download and print this in advance and email or fax it back to our practice. Otherwise simply bring it along with you for your first appointment.
All information is held in strict confidence and will be used to help us at Rosalie Dental to properly assess and develop any treatment plans.
